Transportation Services, Options & Coaching

Transportation needs are not one-size-fits-all. At Onboard Outreach, transportation is approached thoughtfully—through evaluation, guidance, and informed choice—so individuals and families can access transportation that truly supports safety, independence, and participation in daily life.

Transportation services are explored as part of our Supports Brokerage process and are provided as a separate service when appropriate and authorized.

How Transportation Decisions Are Made

Before transportation services are provided, we work with individuals, families, and Support Coordination Agencies to understand needs and determine the most appropriate option.

This evaluation may consider:

  • Mobility, safety, and supervision needs

  • Level of independence and comfort traveling

  • Use of mobility devices or accessibility requirements

  • Consistency versus flexibility needs

  • Approved services, units, and funding

Our goal is not to default individuals into a single model, but to ensure transportation supports real-life goals and daily routines.

Our Transportation Options

Based on evaluation and coordination, individuals may be supported through one transportation option at a time, depending on need, preference, and authorization.

Provider-Operated Transportation Services

Direct transportation services provided by Onboard Outreach for individuals who need consistent, hands-on support, physical assistance, or accessible transportation, following evaluation and coordination.

This option may be appropriate for individuals who:

  • Benefit from consistency and routine

  • Require physical assistance entering or exiting vehicles

  • Use wheelchairs or other mobility devices

  • Attend recurring programs, employment, or appointments

When authorized, Onboard Outreach may provide direct transportation services as a standalone service, with a focus on safety, reliability, and dignity.

Transportation Coordination & Managed Options

Some individuals benefit from greater flexibility and independence. In these cases, we may support transportation through coordination and coaching rather than direct service.

This option may be appropriate for individuals who:

  • Are able to travel more independently

  • Do not require physical assistance

  • Prefer flexible or on-demand transportation

Our role may include coordination, coaching, and administrative support to help transportation work effectively—without acting as the transportation carrier.

Family & Trusted Support Transportation

For some individuals, transportation provided by family members or trusted supports may be the best fit.

This option may be appropriate for individuals who:

  • Rely on familiar supports

  • Have unique schedules or limited transportation availability

  • Benefit from flexible, trusted arrangements

Onboard Outreach may support this option through guidance, coordination, and administrative support, while the transportation itself is provided by the trusted individual.

Important Notes About Transportation Services

Keep in mind...

  • Transportation options are determined through evaluation and informed choice

  • Supports Brokerage and direct transportation services are not provided at the same time

  • Final authorization of transportation services remains with the Support Coordination Agency

  • Transportation needs may be reassessed and adjusted over time

Getting Started

If transportation is a need, the first step is understanding options and determining what fits best.

We invite individuals, families, and Support Coordinators to reach out to begin a conversation about transportation needs, evaluation, and next steps.
