Supports Brokerage

Guiding Informed Choice and Coordinated Support

Supports Brokerage is the foundation of how services begin at Onboard Outreach. We help individuals with disabilities and their families understand options, coordinate supports, and make informed decisions so services work in real life—not just on paper.
 

Our role is to reduce confusion, close gaps between systems, and provide steady guidance as individuals navigate services, transitions, and everyday needs.

What Is Supports Brokerage?

Supports Brokerage is a person-centered service that helps individuals and families:

  • Understand approved services and available options

  • Coordinate multiple supports and providers

  • Navigate systems, paperwork, and follow-ups

  • Identify gaps or barriers and problem-solve solutions

  • Align services with personal goals and daily routines

 

Supports Brokerage is not case management or support coordination. Instead, it works alongside Support Coordination Agencies to help plans translate into meaningful, practical outcomes.

How Supports Brokerage Works

Our Supports Brokerage process is collaborative and individualized. It may include:

1. Understanding Needs and Goals

We begin by listening—learning about priorities, challenges, strengths, and what matters most to the individual.

2. Exploring Options and Services

We help individuals and families understand approved services, supports, and potential pathways forward.

3. Coordination and Follow-Through

We assist with organization, communication, and follow-up so services stay on track and aligned.

4. Ongoing Guidance and Adjustment

As needs change, we help reassess options and adjust supports accordingly.

Supports Brokerage and Transportation

Transportation is often an important consideration, but it is not assumed.


Through Supports Brokerage, we evaluate transportation needs and help individuals and families understand available transportation options. When appropriate and authorized, transportation services may be provided as a separate service, following evaluation and coordination with the Support Coordination Agency.


Supports Brokerage and direct transportation services are not provided at the same time.
