Onboard Outreach, Inc. is a nonprofit organization dedicated to supporting individuals with disabilities and their families through person-centered services that promote independence, dignity, and meaningful community participation.
We exist to help people navigate systems, access the right supports, and move forward with clarity and confidence—especially when services feel overwhelming or disconnected.

At Onboard Outreach, Support Brokerage is the foundation of our work. We help individuals and families understand options, coordinate services, and close gaps between systems so supports are practical, aligned, and effective.
We intentionally operate between systems—working alongside individuals, families, support coordinators, and partner agencies to ensure services connect, follow-through happens, and no one is left navigating alone.
