What We Do
Onboard Outreach provides person-centered support services designed to help individuals with disabilities and their families navigate systems, access resources, and move through life with greater confidence and independence.
We do not believe in one-size-fits-all services. Our work is rooted in understanding each individual’s goals, needs, and circumstances—and providing the guidance, coordination, and support that helps plans turn into real-life outcomes.
Our services are delivered with dignity, clarity, and care, and are designed to complement existing support coordination, not replace it.

Support Brokerage (Core Service)
Support Brokerage is the foundation of what we do.
We help individuals and families understand, coordinate, and manage services, benefits, and life planning needs so nothing falls through the cracks. Our brokers work alongside individuals, families, and support coordinators to ensure services are practical, aligned, and responsive to real-life needs.
Support Brokerage may include:

Understanding and navigating service options

Assistance with paperwork, renewals, and follow-ups

Coordinating services across providers and systems
-
Identifying gaps and problem-solving barriers

Supporting goal planning and next steps
Support Brokerage is especially valuable when systems feel overwhelming, services feel disconnected, or plans are not translating into meaningful progress.
Community-Based Supports
Our community-based supports are designed to help individuals build skills, confidence, and independence in everyday environments.
Services are personalized and may support:

Daily living and life skills

Community inclusion and engagement

Building routines and independence

Strengthening social and functional skills

Supports are delivered in real-world settings and are shaped around each individual’s goals, interests, and pace.


Transportation Services & Coordination
Transportation access is often a barrier to independence. Onboard Outreach provides both transportation services and transportation coordination to help individuals get where they need to go safely, reliably, and with dignity.
Our transportation support may include:

Direct transportation provided by our organization

Transportation coordination with partner providers when appropriate

Access to medical and therapeutic appointments

Transportation to employment, vocational activities, and day programs

Support for community activities and essential errands
Our approach focuses on access, safety, consistency, and respect—ensuring transportation supports real participation in everyday life, not just movement from one location to another.
Resource & Life Navigation
Navigating systems can be complex. We help individuals and families identify, access, and connect to resources that support long-term stability and quality of life.

Training & Workforce Development
Our training division prepares individuals and partners for safe, professional, and rewarding careers. Programs include:

Driver readiness & safety training

Mobility and accessibility training for families
We strengthen the workforce with skills that uplift communities.
