Onboard Outreach is a nonprofit organization dedicated to supporting individuals with disabilities and their families through community-based services, supports brokerage, and transportation services with options and coaching.
We focus on access, dignity, and informed choice—meeting people where they are, evaluating needs, and helping them move forward with confidence, independence, and meaningful support.

Supports Brokerage
Personalized guidance that helps individuals and families understand, coordinate, and manage services, benefits, and life goals—so supports work in real life, not just on paper.
Transportation Services, Options & Coaching
Transportation needs are not one-size-fits-all. As part of our Supports Brokerage process, we evaluate transportation needs first to determine the most appropriate solution based on safety, independence, preferences, and approved services.
We help individuals and families understand available transportation options, review the benefits and limitations of each, and provide coaching and coordination to support the selected approach.
When appropriate and authorized, direct transportation services may be provided as a separate service, following evaluation and coordination with the Support Coordination Agency.
Supports Brokerage and direct transportation services are not provided simultaneously.
*Transportation options are determined through assessment and coordination. Final authorization of services remains with the Support Coordination Agency.
Community-Based Supports
Hands-on supports that help individuals build skills, increase independence, and participate meaningfully in their communities, guided by personal goals and everyday needs.
Resource & Life Navigation
Guidance connecting individuals and families to services, benefits, and community resources—helping reduce overwhelm and support long-term stability and quality of life.

Supports Brokerage is at the heart of what we do. We work alongside individuals, families, and support coordinators to manage the details—services, paperwork, follow-ups, and planning—so individuals can focus on living their lives, not navigating systems.
**Compliance Notice: OnBoard Outreach Inc. is not currently a DDD Medicaid-approved provider. Approval is pending. DDD-approved services are provided through our affiliated provider, Specialty Logistics and Management.
